HOW IT WORKS

All fill-ins are blank: nothing is pre-filled. Write in by hand, or type into the blank spaces with any PDF editor before printing.

TYPE OR WRITE: every blank line is also a fillable PDF field. Open the PDF in free Adobe Acrobat Reader (or Preview on a Mac), click a line and type, then print. Or just print and write in by hand. All fields start empty.

ADD YOUR LOGO: in free Adobe Acrobat Reader on a computer, click any dashed "Your logo" box and choose your logo image (PNG or JPG). Other apps (Preview, phone PDF viewers, browsers) can type in the text fields but may not support adding the logo; you can print and use a logo sticker, or leave the box blank. Only use a logo your company owns or has permission to use.

TEXT STYLE: the fillable fields use a clean standard font (Helvetica).
